An established Hospice organization is currently looking to hire an experienced RN Hospice Case Manager to be the dedicated Hospice Nurse primarily within a large Senior Living Community.

The Lead Hospice Case Manager will work very closely with the campus leadership to ensure a collaborative team approach to providing the absolute best end-of-life care possible to the residents. 

This a well established Hospice that offers great support and resources. The ideal candidate will be an experienced RN Hospice Case Manager with a positive leadership style and a dedication to resident care.
